The FCA was formed as a result of the Global Anglican Futures Conference (GAFCON) in Jerusalem in June 2008 where orthodox and evangelical Anglican Primates, Bishops, clergy and laity unanimously endorsed the Jerusalem Declaration. See gafcon.org
